Let us help you apply for the  Homecare Program

Applying for a Homecare program is one of the essential things a senior or someone with a long-term condition or disability can do for themselves. However, the application process can be extremely tedious and frustrating for someone doing it for the first time. Multiple mandatory steps, assessments, and document submissions must be scheduled, kept, and submitted on time. Fortunately, our Intake coordinators have many years of experience helping others apply for the Homecare program. They guided and supported hundreds of people like you to apply successfully and get approved. Let us help you. We won’t let you down.

Choosing a Caregiver

Choosing a caregiver for yourself or someone you love is not an easy decision to make. Unfortunately, most people are unprepared to make this decision, especially if you require an aide from a home care agency you are meeting for the first time. Thankfully, consumer-option home care in Pennsylvania allows the client to choose a family member or friend to serve as their caregiver. But even then, how do you find the best caregiver?

Here are some tips:

1. You should write down all your needs and discuss them with a coordinator during the assessment. This information is vital for the home care agency in determining your level of care and extremely helpful in deciding who you choose as a caregiver.

2. Write a preferred job description. Is there any specific training you’d like your caregiver to have? For example, should they be able to operate any durable medical equipment you may need? Should they have the ability to speak another language? Again, discuss these things with your homecare agency during your assessment.

3. Prepare additional questions to ask your prospective caregiver. If you don’t know what types of questions to ask, call your home care agency or a referral source.

4. If you choose the caregiver, check their references.

5. Monitor the caregiver’s services and regularly discuss their performance with your Homecare agency. Patient feedback is critical to ensure that the caregiver performs their duties and responsibilities as required and expected.

Choosing your MCO

Community Health Choices is a long-term support program designed to roll all health care, behavioral health care, and long-term services into one extensive plan managed and coordinated by Managed Care Organizations (MCOs). Managed Care Organizations, also known as (MCO) deliver and pay for participants’ services instead of the state.

Anyone receiving Home and Community-Based Services (Home Care program) can choose one of the three Managed Care Organizations (MCOs). Your Homecare agency will work directly with the MCO selected by the participant if they have a contract with that MCO.

Here are the three MCO options:

1) KEYSTONE FIRST
2) UPMC
3) PA H&W
